Working with NHS partners, The Marie Trust delivered an outreach social-prescribing project for people who were disconnected from health and social services.
The project included support from a prescribing pharmacist and focused on reaching people who faced multiple barriers to treatment and ongoing healthcare.
What we learned: Support often needs to be proactive, flexible and available beyond traditional clinical settings.
The Marie Trust developed a partnership with Clyde Munro Dental Group to reduce barriers to oral healthcare for people experiencing homelessness.
The work focused on reaching people who were not registered with a dentist and who faced a greater risk of serious oral-health problems.
The Hospital Outreach Service was developed to support people whose homelessness, housing difficulties or financial hardship became apparent during a hospital stay.
Support can begin on the ward and continue after discharge, helping people access accommodation, benefits, healthcare and other essential services.
Today: Hospital Outreach is one of The Marie Trust’s three principal services.
